Risk Assessment Form Template

A risk assessment form (RAF) contains details of a risk identified during the project. It is a key element of the risk management process*. A RAF states what it is and determines the likely impact, should it occur. It outlines what actions can be taken to mitigate the risk and what controls can be implemented to avoid it.
